If you are interested in getting your child pediatric therapy services, but don’t know where to start, we can get you an appointment for a screening! Your child will get a 15 minute screening, and you will have the opportunity to get professional advice from one of our therapist to learn more about our services and what you can expect from our clinic. If you are interested and/or have recieved a referal for your child to attend Imagine Pediatric Therapies, you can call (479)795-1260 or email info@imaginepediatrics.com, to get a “New Patient Intake” appointment started for you! It’s a 30 minute long appointment, and the child does not have to be present if that is an issue for you. We will give you a packet of the forms provided below, scan your child’s health insurance card, and give you a tour of the facility! Please bring any questions you may have whether it’s about our clinic, therapy services, process of evaluations and treatments, etc.
